Background: The patient was referred by an orthopedic surgeon after experiencing pelvic pain, sciatic-type pain running from her lower back down the left leg and swelling in both legs. She had tried physical therapy and other treatments but did not get relief. She was also diagnosed with a labral tear, but her symptoms were found to be related to significant narrowing (stenosis) of the left common iliac vein.

Diagnosis and Testing: Using a specialized imaging test called intravascular ultrasound (IVUS), we confirmed the narrowing of the left iliac vein, which was affecting blood flow and causing her symptoms.

Treatment: In July, she had a procedure where the vein was opened with a balloon (angioplasty), and a small metal tube (stent) was placed to keep the vein open and improve blood flow.

Results (One Month After Procedure): At her one-month follow-up, the patient reports remarkable improvement, with pelvic and sciatic-type pain mostly or completely gone. The swelling in both legs has also improved. An ultrasound showed that the stent remains open and clear, with no signs of complications.

Plan Going Forward: The patient will continue blood thinning medication for the full three months to prevent clots, even though mild bruising on her left thigh is expected to heal on its own. A routine follow-up ultrasound is planned in three months to check the stent.

Summary: The patient’s pain and leg swelling were caused by a narrowed iliac vein, not just the previously diagnosed labral tear. After opening and stenting the vein, she has experienced significant relief and improved leg swelling. Ongoing care will support continued recovery.
