Background: The patient came to us after struggling with ongoing discomfort in her left leg, even after previous vein treatments (called endovenous ablation). She also described unusual pelvic pain that felt like menstrual cramps, even though she was already post-menopausal. In addition, she had pain during intercourse (a condition known as dyspareunia).
Diagnosis and Testing: We performed a detailed imaging test using intravascular ultrasound (IVUS) and found that the left iliac vein was significantly narrowed, consistent with a condition called May-Thurner Syndrome, which can limit blood flow and lead to symptoms like hers.
Treatment: To restore normal blood flow, we performed a minimally invasive procedure to open the vein using a balloon (angioplasty), followed by placing a stent to keep the vein open.

- Pelvic Pain: The “phantom period” cramps are completely gone.
- Leg Discomfort: Her leg feels significantly better – less heavy, less swollen, and overall more comfortable.
- Daily Life: She’s back to regular exercise and is planning to start traveling internationally again.
- Pain During Intercourse: This has also completely resolved.
Next Steps: She’s doing so well that we’ve transitioned her from a blood thinner (anticoagulation) to a simpler antiplatelet medication. She’ll return in nine months for her routine one-year follow-up and imaging.
