Background: 
This 47-year-old woman first came in with bothersome right leg heaviness, achiness, fatigue and swelling, even though she had very few visible varicose veins. Within just a couple of days after her right great saphenous vein (GSV) ablation in March 2024, she noticed a dramatic change, with the heaviness and fatigue lifting and swelling improving almost overnight.

Current Imaging and Right Leg Plan: 
Her recent vein ultrasound shows that the treated portion of the right GSV, from the groin (saphenofemoral junction) to the knee, remains successfully closed, confirming a durable result. There is some remaining backward flow (reflux) in the lower part of the right GSV below the original treatment area, extending toward the ankle. This will be treated with ultrasound-guided Varithena in the next few weeks to address the remaining abnormal segment.

Left Leg Findings and Treatment Today: 
On the left side, ultrasound confirms vein valve failure in both the left GSV and the anterior accessory saphenous vein, which matches her current symptoms of left leg heaviness and swelling. The plan is to treat these problem veins today with ablation. She understands that a follow-up ultrasound will check for any remaining faulty segments, and that additional Varithena injections may be recommended if needed for full symptom relief.

Clinical Insight: 
This case shows a common pattern of vein disease where symptoms like swelling, heaviness and fatigue are the main complaints, even when varicose veins are not very visible, and how quickly these can improve after treating the main faulty trunk vein. It also underscores the value of staged, stepwise care: first treating the more symptomatic leg, then using follow-up imaging and symptoms to guide when and how to treat the other side.
