Patient: Male, 28 years old
Background: The patient is a healthy 28-year-old firefighter and EMT who experienced heaviness, aching and tiredness in both legs, especially after sitting or standing for a long time. Unlike many people with vein problems, he had no visible varicose veins or skin changes. His symptoms were affecting his physically demanding job and daily life.
Diagnosis and Testing: Although he showed no visible signs, his symptoms led to detailed ultrasound testing (duplex ultrasound) that confirmed venous reflux – where blood flows backward in the veins, causing discomfort and fatigue.
Treatment: In March 2025, the patient had a procedure called endovenous ablation on the right great saphenous vein (GSV), where heat was used to close the faulty vein. After this, his right leg symptoms went away completely. Later, he noticed his untreated left leg was as problematic as before, especially during a long flight, motivating him to return for treatment on his left leg and associated veins.
Results: After the second procedure on his left leg, the patient experienced significant and lasting relief from leg heaviness, aching and fatigue. His quality of life and ability to perform his firefighting duties greatly improved.

- Venous insufficiency can affect young, healthy people without visible varicose veins or skin changes.
- Symptoms alone, like heaviness and achiness, should lead to proper vein evaluations.
- Early treatment can quickly restore function and comfort, especially for active individuals like firefighters.
- Experiencing relief on one side helped motivate treatment on the other side, completing his care.
Summary: This case shows that endovenous ablation is a safe and effective treatment to relieve symptoms of venous insufficiency even in young, active adults without visible vein damage. Recognizing these symptoms early helps improve daily life and work performance for professions with physical demands.
