Background: This 47-year-old healthy woman first presented with very few visible varicose veins but significant right leg heaviness, achiness, fatigue and swelling. Within just a couple of days after her right great saphenous vein (GSV) ablation in March 2024, she felt a dramatic change in that leg, with heaviness and fatigue resolving and swelling improving quickly.​

Current Right Leg Status: Her most recent vein ultrasound shows the previously treated right GSV segment, from the groin (saphenofemoral junction) to the knee, remains successfully closed, confirming a durable result 18 months later. There is some remaining reflux in the lower part of the right GSV below the original treatment site, extending toward the ankle, which will be treated with ultrasound‑guided Varithena in the coming weeks.​

Left Leg Findings and Treatment Plan: On the left side, duplex imaging confirms insufficiency in both the left GSV and the anterior accessory saphenous vein, matching her current symptoms of left leg heaviness and swelling. The plan is to perform left GSV and anterior accessory saphenous ablation today, with the understanding that additional Varithena may be used later if follow‑up ultrasound shows residual problem segments and if symptoms persist.​

Clinical Insight: This case shows a common pattern of chronic venous insufficiency where swelling, heaviness, and fatigue, rather than obvious bulging veins, are the main complaints, yet they can improve rapidly after treating the main trunk vein. It also underscores the value of a staged, symptom‑guided approach: treating the more symptomatic side first, then using interval imaging to guide timing and extent of treatment on the opposite leg.​
