Background:
Chronic venous insufficiency means the veins in the legs aren’t working properly, causing blood to pool instead of flowing back to the heart. This can lead to skin problems like redness, thickening or dark spots long before large, visible varicose veins appear. These early skin symptoms often cause doctors to miss the true problem, delaying proper treatment.
Case Details:
A 41-year-old man came in after years of pain, swelling and skin discoloration in his left leg. Despite seeing several doctors, he hadn’t received a clear diagnosis. He only noticed a visible varicose vein a few months ago, well after the skin changes had started.
Diagnosis and Findings:
An ultrasound of his leg veins showed major backward blood flow (reflux) through the main surface vein on the left leg – the great saphenous vein. This confirmed severe vein valve failure. Deeper veins and larger pelvic veins were working normally.
Treatment:
Doctors performed a quick, minimally invasive procedure called endovenous ablation to seal off the damaged vein using heat. It took about 15–20 minutes and went smoothly.
Follow-Up Plan:
A follow-up ultrasound is scheduled one week later to ensure the treatment worked. If any smaller problem veins remain, the team might use a foam medication (Varithena) to close those as well.
Discussion:
This case shows how vein disease can progress quietly. People may develop severe skin symptoms like inflammation and scarring long before large varicose veins are visible. That’s why it’s important for doctors to think about venous disease early and order ultrasound testing when leg pain or discoloration doesn’t have an obvious cause.
