Patient: Male, 75 years old

Background:
Mr. D. was referred by his neurologist for evaluation of neuropathic symptoms in both legs, worse on the left side. He experienced leg fatigue, heaviness and intermittent tingling, symptoms commonly associated with nerve problems. He also had visible varicose veins and chronic skin changes on his legs.


Diagnosis and Testing:

A detailed ultrasound of his leg veins showed significant reflux (backward blood flow) in the left great saphenous vein (GSV) and anterior accessory saphenous vein (AASV), with moderate reflux in the right GSV. No deep vein problems were found. These findings indicated venous insufficiency causing blood to pool, leading to venous hypertension and swelling.

Clinical Insight:
Mr. D.’s neuropathic symptoms mimic tarsal tunnel syndrome, a nerve compression condition. However, in his case, the nerve irritation is due to venous congestion causing pressure around nerves, especially the posterior tibial nerve in the ankle. This vascular cause of nerve symptoms is often overlooked but can explain numbness, tingling and burning pain in the feet and ankles.

Treatment Plan:
The patient is scheduled for endovenous thermal ablation treatment on his left GSV and AASV, along with a microphlebectomy to remove symptomatic varicose veins. He was informed that improvements in leg fatigue, heaviness and neuropathic symptoms may be noticed within a few days after treatment as the venous pressure normalizes.

Summary:
This case highlights that chronic venous insufficiency can cause neuropathic leg symptoms that look like nerve compression syndromes. Proper diagnosis and treatment of vein problems can significantly improve quality of life by relieving nerve-related pain and discomfort. Interdisciplinary care between neurology and vascular specialists was key to identifying the cause and planning effective treatment.
