If your insurance plan requires a referral from your primary care physician, the referral must be presented at the time of your first visit. Vein Specialists will provide new patient paperwork to be completed and returned 3-5 days prior to your first visit. This allows us time to create and populate your electronic medical record. We can provide these forms in a variety of formats, including e-mail, FAX, and U.S. mail. We also strongly encourage our new patients to use our interactive internet patient portal to complete and submit new patient forms. Simply click on the patient portal button labeled “schedule an appointment” and follow the prompts. The following are just a few points to review at the time you schedule your appointment and before you leave your house on the day of your first or subsequent visit(s).

1. Please check with our staff when you schedule your appointment regarding our network status with your insurance company and other appointment fees.
2. Please bring your current health insurance card(s) and a photo ID with you to each visit at Vein Specialists.
3. If the name on your insurance card does not match the name on your photo ID, we must confirm active coverage prior to your visit. If we are unable to confirm active insurance coverage, you will be asked to make payment in full prior to your consultation, office visit, or treatment. Once the submitted service is remitted, or coverage is confirmed, we will promptly refund the amount due.
4. Please notify the front desk of any changes in your insurance status or other personal information, such as contact information or address.
5. Please notify the medical assistant of any changes in your medical condition, medications, or allergies.
6. Payment of co-pays, deductibles, and co-insurance is required at the time of each office visit.
