Patients with insurance plans requiring primary care physician
referrals must present their referral slip on presentation. Please
let us know how you would like to receive your new patient paper
work (email, facsimile or US mail) at the time you schedule your appointment. Please return this information to the office 3-5 days
prior to your visit to allow our staff adequate time to create
your electronic record. Payment of co-pays, deductibles and
co-insurance is required at the time of each office visit. You will
receive a reminder phone call and/or email 24 hours prior to
you schedule office visit. We kindly request the courtesy of a
return phone call if you will be unable to keep your scheduled appointment. Elective cancellation of sclerotherapy, ultrasound
or operative procedure appointments must be confirmed 24
hours or more before scheduled appointment. Unexcused
missed appointments (sclerotherapy, ultrasounds and vein
procedures) will incur an additional rescheduling fee. We
politely request all accounts be kept current and we reserve
the right to cancel treatment for unpaid account balances.
