Dr. Joseph Magnant participates as an in-network provider
with all BCBS commercial plans, including Medicare
replacement plans. Other in-network plans include United
Healthcare, Aetna, Cigna, Humana, BeechStreet, Lee
Memorial Health Systems (WEB TPA), Medicare and most
other commercial carriers. At this time Dr. Magnant does
not participate with Florida Medicaid or Amerigroup. Please
check with our staff at the time you schedule your appointment
regarding our network status and other office visit fees.
Please bring your current insurance card and a photo form
of identification with you to each visit and notify to receptionist
of any changes in your insurance status or other personal
information.
If the name on the insurance card does not match the name
on your driver’s license, we must confirm active coverage prior
to your office visit. If we are unable to confirm active insurance coverage, you may be asked to make payment in full up front.
Once the submitted service is remitted, or coverage is confirmed,
we will refund the difference.
